Community Input Survey

"Pueblo City Schools District 60 recently received a Creating Healthy Schools Planning Grant. The primary task for this planning period is for the Wellness Task Force to develop a Comprehensive Health & Wellness Plan for the District.

In our first Task Force meeting we reviewed a number of data reports in order to identify the top 9 health concerns within the District. The goal now is to narrow these health concerns to the top 3-5, which will be included in the Comprehensive Health & Wellness Plan.

In order for us to make the best decisions for the District and community as a whole, we need additional community input from students, District staff, parents, and other community members. Please complete this brief survey to help us prioritize the health concerns within our District.
